Pre-Authorization:

We will attempt to obtain prior authorization on all PPO & Worker’s Comp insurances and liens if we are provided with the following information:

• Office Tax ID#
• Clear copy of insurance card (front and back)
• Clear copy of RX with CPT/ICD-10 codes
• Clinical notes – please indicate all related imaging studies
